Sales and Use Tax Contractor's Exemption Certificate (121 kb PDF)

1. Click on a PDF form.
2. Fill out all required fields.
3. Click on the "Submit Form" button in the upper-right corner of the first page.
4. In the box that pops up, fill in your email address and name and click "Send".
5. Choose which email method you prefer.
6. Click "Send" in your email program.
